Chosen Path Community Care Agency is a nonprofit organization focused on strengthening youth, families, and communities through safe housing, supportive programming, educational opportunities, life-skills development, resource coordination, and community partnerships.

Chosen Path operates across both residential and community-based settings, creating opportunities for individuals to receive support according to their circumstances, developmental needs, goals, and service requirements.

The agency's approach recognizes that individuals often require more than one service to achieve stability.

A young person may need housing, education, transportation, mentoring, employment preparation, mental-health resources, life skills, and positive relationships at the same time.

Chosen Path therefore works to create a continuum of support rather than an isolated service.
