Chosen Path Community Care Agency is a community-based organization dedicated to serving at-risk youth through residential care, mentorship, and supportive services. Our agency focuses on providing a stable and nurturing environment for youth ages 14–18, ensuring their physical, emotional, and behavioral needs are met. With 24-hour supervision and trained staff, we emphasize structure, discipline, and personal growth. Our approach combines evidence-based practices with real-life skill building to help youth successfully reintegrate into society and become productive members of their communities.
