Heart of Job Foundation
(House of Job)
The House of Job Program, in collaboration with Chosen Path Community Care Agency, provides structured youth group home placements for young men ages 14–18 who need a safe, stable, and supportive living environment. The program serves youth involved with child welfare, behavioral health systems, or those at risk of instability, delinquency, or homelessness. Participants receive training and exposure in high-demand areas such as technology, graphic design, and music production. In addition to career exploration, youth receive life-skills training, academic support, mentorship, and daily structure. Through coordinated services with Chosen Path Community Care Agency, residents also have access to case management and community-based resources. The program’s goal is to equip young men with valuable skills, confidence, and positive direction for their future.

Make Men Great Again
The Make Men Great Again Program, in partnership with Chosen Path Community Care Agency, provides structured supportive services for young men designed to rebuild character, personal responsibility, and positive identity development. This program focuses on refounding core values such as integrity, masculine manners, care, leadership, support, compassion, love, effort, responsibility, accountability, etiquette, and protection. Participants engage in mentoring, guided discussions, skill-building activities, and structured programming that promotes respectful behavior, emotional awareness, and healthy decision-making. The program serves youth involved with child welfare, behavioral health systems, or those at risk of negative social outcomes. Through coordinated services with Chosen Path Community Care Agency, participants receive access to case management and community-based resources. The overall goal is to help young men develop strong moral foundations, self-discipline, and the character needed to become positive contributors to their families and communities.

Legacy Home Health Group
The Legacy Home Health Group Program, in partnership with Chosen Path Community Care Agency, provides compassionate in-home support for children who require additional care and stability, including autistic children, children with developmental differences, and those facing challenging or disrupted home environments. This collaboration focuses on delivering individualized services that help children remain safely in their homes while receiving the support they need to thrive. Families are provided with guidance, resources, and hands-on assistance tailored to each child’s unique needs. The program emphasizes safety, understanding, emotional well-being, and developmental growth within a familiar environment. Through coordinated care with Chosen Path Community Care Agency, children and families also have access to case management and community-based supports. Together, the partnership works to strengthen families, reduce instability, and promote positive long-term outcomes for children.
