Chosen Path Community Care Agency provides comprehensive, youth-centered services designed to support the development, stability, and success of each individual in our care. Our residential group home program offers a safe, structured environment with 24-hour supervision, ensuring that all youth receive consistent guidance and support. We provide individualized care planning tailored to each youth’s behavioral, emotional, and developmental needs, along with daily life skills training focused on responsibility, decision-making, communication, and independence.

In addition to residential care, we offer mentorship and counseling support to help youth navigate personal challenges, build healthy relationships, and develop positive coping strategies. Our programs include educational support, workforce readiness training, and structured recreational activities that promote personal growth and teamwork. Through partnerships and community-based initiatives, we also provide diversion services aimed at helping youth involved in the juvenile system successfully reintegrate into society.

Youth Group Home
Chosen Path Community Care Agency provides structured, 24-hour residential care for youth ages 14–18 in a safe and supportive group home environment. Our program is designed to promote stability, accountability, and personal growth through daily routines, supervision, and individualized care plans. Youth receive guidance in behavior management, conflict resolution, and independent living skills while being supported by trained staff dedicated to their overall well-being and success.
Case Management
Chosen Path offers comprehensive case management services to ensure each youth receives coordinated and individualized care. Our case managers assess needs, develop service plans, monitor progress, and connect youth with essential resources such as education, healthcare, and community programs. We also work closely with families, courts, and other agencies to ensure consistency, accountability, and successful outcomes.
Family Services
Chosen Path believes family involvement is key to a youth’s success. Our family services are designed to strengthen relationships, improve communication, and provide support to parents and guardians. We offer family counseling, education, and resources to help create a more stable and supportive home environment, both during and after a youth’s time in our program.
Therapy
Our therapy services focus on addressing the emotional, behavioral, and mental health needs of each youth. Through individual and group sessions, we help youth process trauma, develop coping strategies, and build healthier thought patterns and behaviors. We work with qualified professionals and evidence-based approaches to ensure each youth receives the support needed to heal, grow, and move forward in a positive direction.
Youth Placement
We provide youth placement services in collaboration with courts, agencies, and referral partners to ensure youth are placed in safe, appropriate, and supportive environments. Our placement process focuses on matching each youth with the right level of care based on their needs, background, and goals. We prioritize stability and structure to support successful transitions and long-term development.
Community Support (Pantry, Speaking Engagements, Closets)
Our community support services extend beyond residential care to meet the broader needs of families and individuals in the community. We provide access to essential resources such as food through our pantry, clothing through our community closets, and educational support through speaking engagements and outreach programs. These services are designed to uplift the community, promote awareness, and provide practical assistance to those in need.
